Important Patient Information

What to Know Before Your Appointment

We believe in building long-term relationships and keeping treatment plans conservative.

At the first visit, we need to be aware of your medical history, any medical conditions your may have that require pre-medication for treatment, any prescriptions you are taking, any drug allergies, and any concerns you may have. We would like to know what you like and don't like about your current smile.

We believe in evaluating your entire dental needs and therefore recommend a full set of radiographs to prevent any future emergencies. We use digital radiography to decrease the radiographic exposure. We also evaluate for tooth decay, bone loss, gum disease, TMJ problems, and oral cancer or any other pathology.

We accept multiple insurances including Met Life PDP- Active Military, United Concordia PPO, Delta Dental (Premier), Ameritas, Humana-PPO, Aetna PPO, Guardian PPO, Dentemax and Careington, and United Healthcare. If we are not a provider for your insurance, we can still see you in most circumstances.
Dental insurances often cover portions of services and the remainder is the responsibility of the patient.

We accept cash, checks, credit cards including Mastercard, Visa, Discover, and American Express. We also accept Care Credit, which can be interest free. We also provide payment plans for more extensive treatment plans.
